When the form is coded, staff can submit the form via a ticket request to the Support Portal. Please reach out to ticket submitters at your county if you do not have a Support Portal account.
Here are some helpful tips for submitting a ticket:
- Provide a blank copy of the county specific form and a coded copy of the form when submitting a new form to be developed.
- Please ensure the blank copy is a editable version. Submitting images or scanned in copies will be harder to modify the layout for Forms Team.
- Identify a title for the form when submitting a ticket for New Forms Requests.
- You can provide brochures and informational documents as is if these do not need coding.
- Provide a taxonomy for the form. Taxonomy lists is found within the AutoFill Form Codes template. Please choose a taxonomy within the CaseWorks Edition (ACE, CSE, FSE/MSE, SSE, WFE).
- You do not need to provide an existing county specific form if you are making small tweaks (i.e. change the date to a calendar, add a textbox etc.). This can be communicated in the ticket by referencing the existing form. If there are major changes to the existing county specific form, we recommend to recode the form.
